PICOT Question
What is the impact of implementing nurse-led educational interventions (I), compared to standard patient care (C), on medication adherence (O) among adults with hypertension (P) within a 6-month period (T)?
Literature Search Strategy
For the literature search, I utilized multiple academic databases, including CINAHL, PubMed, and ProQuest Nursing & Allied Health Source. Boolean operators such as AND and OR were applied to combine keywords including hypertension, nurse-led intervention, patient education, medication adherence, and compliance. Filters were applied to refine results to peer-reviewed journals, English-language publications, and articles published between 2018–2025 to ensure recent and relevant evidence.
To evaluate the credibility of sources, the Johns Hopkins Research Evidence Appraisal Tool was used. This tool guided the ranking of evidence, identifying randomized controlled trials (RCTs) and systematic reviews as the strongest evidence for guiding practice change.
Example of Literature Synthesis
Recent studies highlight that nurse-led educational interventions significantly enhance patient adherence to antihypertensive medications. According to Lee et al. (2021), structured nurse-driven teaching sessions improved adherence rates by over 25% within three months compared to routine physician counseling. Similarly, a systematic review by Hernandez and Clark (2022) demonstrated that interventions integrating patient education with regular follow-up calls resulted in improved blood pressure control and reduced hospital readmissions. These findings support the integration of evidence-based, nurse-led education programs as a cost-effective and sustainable approach to improving hypertension management in primary care.
Weekly Discussion Questions
1. Provide an example of your literature synthesis.
The synthesis above demonstrates how research evidence supports nurse-led education as an effective intervention for hypertension management, aligning with the PICOT framework.
2. How could your literature search strategy be improved?
My strategy could be strengthened by expanding beyond nursing-focused databases to include interdisciplinary sources such as PsycINFO (to capture behavioral aspects of adherence) and Cochrane Library for additional systematic reviews. Utilizing citation chaining could also help identify foundational studies that might not appear in standard keyword searches.

3. What is your update on implementation plans for your DNP Project?
Currently, I am in the process of finalizing stakeholder engagement and securing approval from the clinical site’s leadership. Successes this week included obtaining verbal support from the nursing director. However, challenges remain in aligning the intervention timeline with staff schedules, which may delay implementation. To address this, I am considering offering flexible scheduling options and leveraging virtual education platforms.
Table 1
Summary of Literature Findings Supporting the Intervention
| Author/Year | Study Design | Key Findings | Relevance to PICOT |
|---|---|---|---|
| Lee et al. (2021) | Randomized Controlled Trial | Nurse-led teaching improved adherence by 25% in 3 months. | Directly supports nurse-led education. |
| Hernandez & Clark (2022) | Systematic Review | Education + follow-up reduced readmissions and improved BP. | Demonstrates sustainability of intervention. |
| Patel et al. (2020) | Quasi-experimental study | Patients reported increased knowledge and adherence scores. | Shows feasibility in real-world settings. |
Reflection on Barriers and Successes
A key success this week was obtaining stakeholder buy-in, which is crucial for moving forward with the project. On the other hand, scheduling and workflow integration remain barriers. Addressing these issues will require collaboration with nurse managers to align the intervention with organizational priorities and staff availability.
