NURS FPX 4050 Assessment 4 Final Care Coordination Plan

Student Name Capella University NURS4050 Coord Patient-Centered Care Prof. Name Date Final Care Coordination Plan This care coordination plan is based on evaluating the preliminary care coordination plan developed in Assessment 1 for mental health issues in the Lodi Community. This plan focuses on developing patient-centered interventions considering ethical implications, health policy provisions, and Healthy People 2030 objectives using evidence-based research. Mental health is the psychological and emotional well-being of individuals that assists them in addressing stressful life situations and recognizing their abilities to contribute positively to the community (WHO, 2022).  Patient-centered Health Interventions and Timelines for Mental Health  Various issues under the domain of mental health must be eradicated using patient/population-centered interventions. These issues are; limited mental health literacy, inadequate community support programs, and mental health issues in school-going children and adolescents. Mental Health Literacy/Education A patient-centered intervention to address this concern is the implementation of educational campaigns. The purpose of these campaigns is to disregard the stigma associated with mental health conditions and promote sharing and discussions related to mental well-being. These campaigns will be conducted in the form of awareness sessions. Social media applications will be used for a wider spread of information, as they are considered effective in promoting mental health awareness (Latha et al., 2020). The timeline for this intervention is divided into two sections – 1) development of the application, which will be done within three months, and 2) awareness sessions, which will be continuously conducted once per month. Furthermore, community resources like mental health advocates, community care centers, and psychological well-being clinics will be utilized to achieve the desired objectives.  Community Support Networks Another important healthcare challenge is inadequate community support resources. The establishment of community support groups can address this challenge. The goal is to create at least five support groups within the year’s timeline. Effective mobilization of community stakeholders such as leaders, faith-based organizations, financial resources, healthcare organizations, and governmental support is essential for fulfilling the community-based initiative (Siddiqui et al., 2022).  School-based Mental Health Programs A patient-centered intervention is establishing a school-based mental health curriculum to address the rising prevalence of mental health issues in children and adolescents. This educational intervention is vital to minimize the risks of mental health issues in the concerned population, early detection of signs, which is important for early intervention, and improve mental health literacy from an early age (Richter et al., 2022). The timeline for this intervention is that the planning should begin within a month, the course should be developed in 3 months, and the execution of the curriculum should begin within six months. Initially, a pilot project should be undertaken for 3-6 months, which after evaluation and modification, should be implemented in all schools. Educators, school administrators, and healthcare professionals in the mental health domain are major community resources required for this project.  Ethical Decisions Related to Patient-centered Health Interventions Ethical considerations are crucial when designing patient-centered health interventions related to mental health issues due to the associated stigma. Cultural sensitivity and acceptance of diversity are two major ethical concepts that must be considered while developing educational/awareness campaigns for the community. It is imperative to develop culturally sensitive content that recognizes the unique mental health challenges faced by various racial/ethnic groups within the community. Besides, using social media applications raises ethical concerns about privacy and confidentiality (Ahmed et al., 2020). NURS FPX 4050 Assessment 4 Final Care Coordination Plan In this regard, it is crucial for healthcare providers to adhere to the rules of maintaining patients’ privacy and confidentiality during the implementation of social media for information-sharing purposes. Establishing community support networks comes with the ethical principle of justice whereby every individual member should receive equitable community resources and fair treatment. A study by Creary (2021) suggests that applying justice and efficiency in terms of the allocation of resources can lead to health equity and minimize disparities. Lastly, developing a school-based mental health curriculum involves the ethical principles of autonomy and informed choices. Students/parents should be given the autonomy to decide what is best for their healthcare based on the curriculum’s evidence-based informed risks and benefits.  Coordination and Continuum of Care Related to Mental Health – Healthcare Policies Several policies significantly impact care coordination related to addressing mental health issues in communities.  Mental Health Parity and Addiction Equity Act (MHPAEA) This health policy was developed in 2008 to reduce inequalities and disparities in accessing mental health services. Further amended under the Affordable Care Act (ACA) policy, this act instructs health insurance providers to give equal coverage to mental health and substance abuse concerns as provided for other physical health conditions (CMS, n.d.). The provisions of this policy improve healthcare accessibility for patients with mental health illnesses reducing financial barriers, leading to early interventions and improved quality of life through coordination of care.  21st Century Cures Act The Cures Act aims to enhance health outcomes for individuals with mental health and substance abuse issues, especially in the criminal population. This act reduces racial disparities and provides equal opportunities for the concerned population so that the continuity of care is maintained and individuals receive equitable access to healthcare (Cole et al., 2018). Furthermore, this legislation also provides funding for mental health initiatives, encouraging coordination and integration of mental health services within primary care settings. Priorities and Evidence-based Changes in Care Coordination While engaging in discussions with patients and their families, the care coordinator must establish the priorities to ensure effective communication of the care plan. Comprehensive assessment and goal-setting is an initial priority where the care coordinator will assess patients’ physical, mental, and psychosocial needs. Engaging the patient and their family member in dialogue allows the coordinator to gain valuable insights into the patient’s medical history, current symptoms, and personal preferences. This approach is essential for providing patient-centered care and establishing realistic and meaningful goals. The need for changes to the plan arises from evidence-based practice. These practices emphasize the importance of individualized care plans targeting specific goals to optimize health outcomes (Bolton

NURS FPX 4050 Assessment 3 Care Coordination Presentation to Colleagues

Student Name Capella University NURS4050 Coord Patient-Centered Care Prof. Name Date Care Coordination Presentation to Colleagues Good morning colleagues. I welcome you all for today’s presentation on coordinated care and basic concepts revolving around the topic. This session aims to provide essential knowledge about care coordination, practical strategies for collaborating with our patients and communities, change management strategies, and highlight ethical-decision making and the impact of healthcare policies on care coordination. Moreover, we will discuss nurses’ crucial role in maintaining the continuum of care. Let us move ahead in the presentation.  Evidence-based and Culturally-sensitive Strategies for Collaboration  First, we will discuss research-based and culturally appropriate strategies to collaborate with patients and their families in community settings effectively. These strategies are; shared-decision making and culturally competent care.  Shared-decision Making A patient and family-centered approach that revolves around collaborative discussions between providers, patients, and their families about patients’ healthcare treatment options, care plans, and objectives will assist in making informed choices (AHRQ, 2020). This strategy is implemented by providing evidence-based information, which includes risks and benefits associated with the treatment options to promote informed decision-making. This strategy aims to respect patients’ rights, cultural values, and preferences and ensure that patients’ views are equally important in managing health conditions. Cultural Competence Another effective way to collaborate with patients and their families is to provide culturally competent care. For this purpose, healthcare providers must develop cultural competence by recognizing and respecting patients’ cultural values, practices, and beliefs. Moreover, using appropriate language to convey healthcare information will enhance patients’ trust, and tailoring treatment plans according to patient’s needs and preferences will improve patient satisfaction levels. A study by Handtke et al. (2019) claims that cultural competence can reduce disparities and increase consumer satisfaction, eventually improving healthcare quality.  Change Management and its Impact on Patient Experience The change management strategies significantly impact the healthcare industry, directly affecting patient experiences and quality of care.  Two different concepts must be distinguished for better understanding – patient experience, which incorporates a variety of interactions that patients encounter during their care period in a healthcare organization. It is considered an important dimension that depicts the quality of healthcare services provided to patients (Oben, 2020). The other is patient satisfaction, which indicates healthcare providers’ success in meeting the patient’s needs and desired objectives.  Effective communication, comprehensive care, and care coordination are critical aspects of change management that directly impact patient experiences. Clear and open lines of communication between healthcare providers and patients are pivotal in enhancing patients’ trust in the healthcare services and engaging patients in their care by informing them about health conditions, treatment options, and further progress of the diseases (Kwame & Petrucka, 2021). Additionally, appropriate communication techniques and frequent correspondence with patients and their families can result in desired outcomes, enhancing patient satisfaction levels. NURS FPX 4050 Assessment 3 Care Coordination Presentation to Colleagues Another essential aspect of expanding patient experience is personalized and comprehensive care which involves shaping care plans according to patients’ needs, cultural values, and preferences. As personalized care shows commitment to individual needs, it significantly improves patient satisfaction. Lastly, smooth transitions between the care (coordinated care) reduce perplexity for the patients and minimize the disruptions within the care process, improving their experience with healthcare services (Figueroa et al., 2018). Care coordination also assists in bridging the gap within the care, preventing delayed treatments, and improving patient satisfaction. To sum up, effective change management should spotlight patient-centered approaches that increase the patient experience by addressing the needs and preferences of the patients.  Coordinated Care Plans and Ethical Decision Making Coordinated care plans are a fundamental aspect of healthcare developed to provide holistic patient care. When based on ethical principles, these plans are essential to provide patient-centered care, enhance communication, maintain continuity, and allocate resources equitably. Ethical decision-making prioritizes patient-centered care under the principle of autonomy. Similarly, coordinated care plans guarantee interprofessional collaboration to meet the needs of the patients tailoring the care plans according to their preferences (Lulé et al., 2019). NURS FPX 4050 Assessment 3 Care Coordination Presentation to Colleagues Moreover, coordinated care plans prevent fragmented care and assist healthcare providers in working for patient benefit by reducing adverse effects. This aspect is based on the ethical principle of beneficence and non-maleficence. Ethical principles guide the fair allocation of resources and equitable access to care (O’Sullivan et al., 2022), which is the foundation for care coordination as it helps reduce disparities due to various social, cultural, and economic factors. The care coordination plans result in positive consequences of enhanced communication, improved patient outcomes, continuity of care, and adequate allocation of resources. Underlying assumptions guiding ethical decision-making are patient satisfaction and empowerment, which are essential for engaging patients in their care processes. Moreover, it is assumed to provide multifaceted care through ethical decision-making, an essential aspect of care coordination.  Impact of Health Care Policies on Health Outcomes and Patient Experiences Policies made in the healthcare sector have a significant impact on patient experiences and health outcomes. Similarly, the Affordable Care Act (ACA) is a policy based on expanding Medicaid eligibility, which ultimately has increased healthcare accessibility for millions of uninsured individuals. A study based on the impact of Medicaid expansion on older adults revealed that this expansion has reduced morbidities, complications, and risks associated with various diseases. It improved the physical health of low-income individuals (McInerney et al., 2020). These improved health outcomes have also enhanced patient experiences with care and patient satisfaction levels. Additionally, the ACA’s emphasis on value-based care and payment reforms has incentivized healthcare providers to ensure that care quality is always prioritized, fostering care coordination.   NURS FPX 4050 Assessment 3 Care Coordination Presentation to Colleagues Another policy that has impacted patient experiences and health outcomes is Health Insurance Portability and Accountability Act (HIPAA). HIPAA provisions are significant in safeguarding the privacy and security of patient information. A study reveals that privacy is the cornerstone of a patient-provider trusting relationship (Iott et al., 2020). Thus, the security of the patients’ information,

NURS FPX 4050 Assessment 2 Ethical and Policy Factors in Care Coordination

Student Name Capella University NURS4050 Coord Patient-Centered Care Prof. Name Date Ethical and Policy Factors in Care Coordination Greetings, everyone. I am  ______, a care coordinator at Lodi community care center. Today’s presentation concerns the ethical and policy factors influencing care coordination within community settings. As representatives of various homeless shelters, this presentation is essential for you all as it will highlight the concept of care coordination, governmental policies affecting care coordination, ethical considerations, and health policies tailored for the homeless population to maintain their continuum of care. Lastly, the impact of the nursing code of ethics will be discussed.  What is Care Coordination?  Moving ahead in our presentation, let us dig into the concept of care coordination. Care coordination is strategic patient/population care planning and appropriate dissemination of protected health information. It is an approach used to provide patient/population-centered care to assess and accomplish the health needs of concerned individuals (Karam et al., 2021). There are various ethical considerations and health policies which influence care coordination. Let us discuss some of them ahead.  Governmental Policies Affecting the Care Coordination Several governmental policies significantly promote the health and safety of vulnerable populations, especially homeless people. These policies under federal/state level government assist homeless shelters to follow the standards of practices defined by the policies regarding healthcare accessibility and safety of the concerned population.  NURS FPX 4050 Assessment 2 Ethical and Policy Factors in Care Coordination Care coordinators are vital in effectively implementing these policies within and outside homeless shelters to ensure that the homeless population receives equitable healthcare opportunities and their safety is protected. Next, in our presentation, we will discuss the policies that raise ethical dilemmas in coordinated care.  Ethical Dilemmas for Care Coordination  Despite the positive impacts of the policies on care coordination, some ethical issues may arise during the implementation of these policies. The four major principles of ethics are – autonomy, beneficence, non-maleficence, and justice. These principles generally apply in all circumstances, especially in the healthcare domain.  One significant ethical issue in the healthcare sector is data protection – privacy, security, and confidentiality. Care coordination, especially in homeless shelters, involves collecting and sharing persons’ protected health and sensitive information. Several ethical questions arise related to the privacy and confidentiality of this information. According to the Health Insurance Portability and Accountability Act (HIPAA), it is imperative to maintain the privacy and confidentiality of sensitive information and only share it with authorized personnel upon receiving consent from the individuals (Moore & Frye, 2019). Balancing the importance of effective care coordination with protecting individual privacy rights is crucial and may raise ethical dilemmas for healthcare providers. Another ethical dilemma is the criminalization of homelessness. Several cities and states in the U.S. have employed policies that criminalize behaviors like sleeping on the streets and trespassing. Such policies raise ethical questions about an individual’s dignity (Rankin, 2019). These policies may impose legal implications and arrests for homeless people, raising ethical dilemmas and preventing individuals from receiving adequate healthcare services.  Impact of Nursing Code of Ethics  The nursing code of ethics is significant in forming care coordination and maintaining the continuum of care for susceptible populations, including homeless people. These ethical principles enable nurses to enhance their coordination while addressing healthcare disparities and ethical concerns keeping in mind the social determinants of health (SDOH). Healthy People 2030 defines SDOH as economic stability, education and literacy, healthcare access and quality, social and community factors, and environmental factors (U.S. Department of Health and Human Services, n.d.). NURS FPX 4050 Assessment 2 Ethical and Policy Factors in Care Coordination When dealing with the homeless population, nursing professionals can consider these factors; following the policies for healthcare accessibility and using ethical principles can reduce inequalities and disparities among the homeless population. Moreover, the nursing code of ethics emphasizes that professionals advocate for the well-being of the communities. This advocacy can modify policies currently preventing homeless people from acquiring healthcare services. Additionally, the code of ethics focuses on the provision of culturally-competent healthcare. Stigma and discrimination encountered by the homeless population can be disregarded by observing the ethical codes of justice and equitable healthcare (Sadeghi et al., 2022). Therefore, it is crucial for nurses to be aware and educated about the code of ethics so that community individuals can receive effective and adequate care preventing health disparities.  Conclusion To summarise the presentation, I would like to emphasize the need for care coordination in the context of the homeless population. Several policies, like Affordable Care Act and the Housing First policy, positively impact care coordination; however, some ethical issues may arise during the implementation of coordinated care. Thus, it is imperative for healthcare providers, especially nurses, to get educated about the code of nursing ethics, as abiding by these ethical principles will ensure the effective and adequate provision of coordinated care. NURS FPX 4050 Assessment 1 Preliminary Care Coordination Plan

Student Name Capella University NURS4050 Coord Patient-Centered Care Prof. Name Date Preliminary Care Coordination Plan Care coordination is described as a holistic approach, which includes interdisciplinary collaboration to provide patient and family-centric care within the healthcare and community settings. Taking the expanded role of case management staff in Lodi Community Care Center, the focus of this assessment is to address the mental health concerns in the community. World Health Organization (2022) describes mental health as psychological and emotional well-being that assists individuals in addressing life stresses, recognizing their potential, and contributing positively to the community. Like physical health, mental well-being is also integral to human health. Communities must take appropriate actions to address such issues if observed. This assessment focuses on analyzing mental health concerns and best practices for improving community health, developing SMART goals, and identifying resources for care coordination. Analysis of Health Concern and Best Practices for Improving Health Mental health issues have extended significantly in recent years due to their persistent impact on individuals and communities. It is estimated that 57.8 million people in the United States have mental health problems (U.S. Department of Health and Human Services, 2023). Several communities often stigmatize mental health issues, leading to underreporting and inadequate support for concerned individuals. Common mental health issues include stress and depression, anxiety disorders, schizophrenia, and substance abuse problems. These issues affect the individual’s emotional well-being, physical health, relations, productivity, and overall quality of life. Some evidence-based best practices for improving mental health are; mental health education, therapeutic care approaches, and community support programs.  NURS FPX 4050 Assessment 1 Preliminary Care Coordination Plan Assumptions and Uncertainties The underlying assumption in this analysis is that mental health is equally important as physical health, which is why evidence-based best practices should be utilized to improve community health. The areas of uncertainty are; the community’s perception of discussing mental health concerns, commitment from community leadership in bringing reforms, and individuals’ cultural and social barriers to addressing the challenges.  Specific Goals for Addressing Mental Health Issues To effectively address the mental health challenges within the community setting, it is imperative to develop realistic, measurable, and attainable goals. The following goals are established for our community.  Goal 1 – Increase mental health awareness and literacy The goal is to implement educational campaigns about various mental health issues, alleviate stigma, and promote the culture of sharing and discussing mental well-being. The aim is to increase mental health literacy by 70% among the community members and increase their willingness to engage in open discussions about particular health aspects. Social media and various apps are considered effective in promoting and creating mental health awareness as the use is widespread and minor information can be disseminated in a shorter period (Latha et al., 2020). Furthermore, collaboration with mental health support groups, community care centers, and healthcare organizations can be another way to design awareness campaigns.  Goal 2 – Implement school-based mental health programs Since school-going children and adolescents are at high risk for developing mental health issues, the establishment of school-based mental support programs is significant to reduce the number of incidences, recognize the signs of distress among the concerned population, and improve mental health literacy from early ages of life (Richter et al., 2022). The goal is to observe a 40-50% reduction in the rate of mental health illnesses among children and adolescents. For this purpose, mental health curriculum and a partnership between local schools and healthcare providers will be established to deliver the curriculum effectively. Moreover, mental health counseling services and support groups will be provided to affected individuals.  Goal 3 – Enhance community support networks Establishing support groups within the community will ensure that individuals dealing with mental health issues have a safe space to discuss their concerns, implement coping interventions to alleviate challenges, and improve their well-being. This is only possible if community and faith-based leaders actively commit to implementing the change. The goal is to develop at least five support groups by the end of the year and ensure members’ participation through data monitoring. In this regard, collaboration with mental health institutions, community health centers, and religious organizations is essential to facilitate the setup. Moreover, support from healthcare providers and mental health specialists is fundamental for the successful operation of these support groups. Community Resources for Care Coordination A holistic care coordination plan for mental health issues in community settings must be developed utilizing community resources so that the continuum of care is established and individuals are aware of support sources in crises. One such resource for adequate management of mental health concerns is mental health organizations. According to CDC, various organizations provide mental health services to community individuals. These organizations are the American Psychiatric Association Foundation, the American Psychological Association, and the American Academy of Child and Adolescent Psychiatry. These organizations provide various mental health support programs, psychiatrists and psychologists’ services, and educational resources, which can be used for awareness, treatment, and management purposes (CDC, 2023). Another community-based resource is helplines/hotlines, which provide immediate assistance to individuals in mental health crises. They are offering emotional support and guidance on accessing appropriate care. NURS FPX 4050 Assessment 1 Preliminary Care Coordination Plan One of the national helplines is the Substance Abuse and Mental Health Services Administration (SAMHSA) (U.S. Department of Health and Human Services, 2022). Other than these, resources like faith-based support groups, online mental health platforms (BetterHelp, Talkspace), school-based counselors, and non-profitable organizations (NPOs) are some accessible resources for community individuals to utilize for mental health services.  Conclusion In conclusion, mental health issues are significant health challenges within community settings due to stigmatization and non-acceptance within society. For this purpose, preliminary care coordination plans are essential to improve the mental well-being of community members. A similar care plan has been developed for the Lodi community. The goals are to increase mental health literacy, implement school-based programs, and ensure accessible and adequate community resources available for the members. Some U.S.-based community resources that can be utilized are national mental
