Final Preceptor Evaluation
Purpose
The primary purpose of this assignment is to ensure that the preceptor evaluates and provides constructive feedback regarding the learner’s performance and achievements throughout the practicum experience. This feedback serves as a reflection of the student’s growth, strengths, and areas for improvement. Additionally, it allows both the preceptor and the student to assess how well the practicum objectives were met and to identify opportunities for continued professional development.
Instructions
The preceptor will receive the Final Evaluation form through Medtrics, accompanied by step-by-step instructions. The form becomes available ten days before the due date to allow adequate time for completion. Once filled out by the preceptor, the evaluation form will be accessible to the student under the “Evaluations” section in Medtrics.
To complete this assignment, follow these steps:
- Access the Evaluation:
- Log in to Medtrics.
- Select “Evaluations” from the left-side navigation bar.
- Locate Your Evaluation:
- Under the My Evaluations header, click “About Me” in the upper left.
- Choose “View” from the bottom right of the screen to review your evaluation form and add necessary comments.
- Finalize the Evaluation:
- Scroll down to the bottom of the page, select Submit, and electronically sign the evaluation.
- Download and Save:
- Click “PDF” at the bottom right (next to “View”) to download the evaluation document.
- Save the document on your device.
- Submit in Canvas:
- Upload the saved PDF into the designated Canvas assignment portal.
