Understanding Epidemiology
Epidemiology refers to the systematic study of how various determinants of health and disease are distributed within human populations. It is a core discipline in public health that not only examines the occurrence of disease but also identifies factors influencing health outcomes and the patterns through which diseases spread or diminish. Through epidemiological studies, health professionals can design targeted interventions, evaluate disease control measures, and improve population health outcomes (Friis & Sellers, 2021).
Person–Place–Time Model
The Person–Place–Time Model is an essential framework in epidemiology that addresses three critical questions:
- Who is affected? (Person)
- Where does the condition occur? (Place)
- When does it occur? (Time)
This model helps in identifying high-risk groups, geographic distribution of diseases, and seasonal or temporal trends.
The Agent–Host–Environment Interaction
The agent represents the cause of the disease, which can be biological, chemical, physical, or nutritional in nature. Host factors refer to individual characteristics that influence susceptibility or response to the agent, such as age, genetics, and prior immune status. The environment includes all external conditions affecting the agent and host, such as physical surroundings, socio-economic conditions, and biological ecosystems.
Examples of Agents and Host Factors

Environmental factors can be:
- Physical (climate, geography)
- Biological (human, animal, and plant populations)
- Socioeconomic (occupation, urbanization, economic development, and social disruption)
Epidemiological Triangle
The Epidemiological Triangle visualizes the interaction between agent, host, and environment, emphasizing that disease results from the complex interplay of these three components.
Ecosocial Approach
The ecosocial model integrates microbiological processes with evolving socio-environmental and economic contexts to explain patterns of health and disease. This perspective emphasizes that individual health outcomes are influenced not only by pathogens but also by macro-level factors such as globalization, urbanization, and social inequality.
Calculating Rates in Epidemiology
In epidemiology, rates help quantify disease occurrence:
- Example: If 500 students are screened for tuberculosis (TB) and 15 test positive, the incidence rate is:
15500=0.03 (3%), or 30 per 1000 students at risk.\frac{15}{500} = 0.03 \ (3\%), \ \text{or} \ 30 \ \text{per 1000 students at risk}.

Six criteria help establish causality:

- Temporal relationship (cause precedes effect)

- Specificity of the association

Periods in Disease Development
- Latent period: The pathogen has invaded and begun replicating but is not yet transmissible.
- Communicable period: Begins when the pathogen can be shed and transmitted, following latency.
- Incubation period: Time between pathogen invasion and symptom onset, which may overlap with communicability.
Chain of Transmission
The transmission of communicable diseases occurs through a six-link chain:

- Acquired Immunity
- Active: Achieved via vaccination.
- Passive: Gained through monoclonal antibodies or blood products.
Vaccine Failures
- Primary vaccine failure: The body fails to respond to a vaccine initially.
- Secondary vaccine failure: Immunity is lost over time after initially developing it.
Case Classification
Diseases may be classified based on:
- Confirmed cases
- Probable cases
- Laboratory-confirmed cases
- Clinically compatible cases
- Epidemiologically linked cases
- Genetic typing results
- Clinical case definitions
Immunization vs. Vaccination
Immunization refers to the broad process of inducing or enhancing immunity, either naturally or artificially, while vaccination specifically means the administration of a vaccine or toxoid to develop active immunity.
Contraindications for Vaccination
Individuals who may be advised against vaccination include those who are immunocompromised, pregnant, or suffering from certain acute illnesses.
